The reason why an interview may be scheduled but not automatically have the feedback forms populated is because the specific interview template does not have a default feedback form yet. The default feedback form for each interview template event is based on the feedback forms selected during the last time a job and stage combination was used in an interview that was created and confirmed manually.

If the interview template has never been used for a specific job and stage combo before, then there is no default feedback form for the events. This means that the feedback form fields will be empty so it'll have to be manually assigned by editing it after creation.

Please note that editing the feedback forms after creation will not assign them as default feedback forms. An interview using a template and a specific job combo really needs to be manually created and confirmed through the web app using the Request Availability or Schedule Now flow to set the default forms.
